Commit 99bcad18 deleted ADCIOP and DASA_SIM board support
but missed to delete board/esd/adciop and board/esd/dasa_sim.
It also missed to add entries to doc/README.scrapyard.

This patch moves some ppc4xx related headers from the common include
directory (include/) to the powerpc specific one
(arch/powerpc/include/asm/). This way to common include directory is not
so cluttered with files.

instead CFG_MONITOR_LEN is now only used to determine _at_compile_
_time_ (!) if the environment is embedded within the U-Boot image,
or in a separate flash sector.
* Cleanup CFG_DER #defines in config files (wd maintained only)
